Janma-Mrityu Tathya Portal

The Janma-Mrityu Tathya Portal is an online initiative by the Government of West Bengal aimed at simplifying the registration and issuance of birth and death certificates for residents of the state. This digital platform ensures transparency, reduces manual errors, and provides citizens with easy access to vital records.

How to Use the Portal

For Birth Registration

  1. Visit:  janma-mrityutathya.wb.gov.in
  2. Navigate: Go to Citizen ServicesBirthApply for New Registration.
  3. Register: Enter your mobile number and verify it using the OTP sent.
  4. Fill Form: Complete the application form with accurate details.
  5. Upload Documents: Attach necessary documents such as hospital discharge certificate, Form-1 (for home births), and ID proofs of parents.
  6. Submit: Submit the application. An acknowledgment number will be provided.
  7. Follow-Up: Visit the respective hospital, nursing home or municipal office with the acknowledgment number to collect the certificate.

 For Death Registration

  1. Visit:  janma-mrityutathya.wb.gov.in
  2. Navigate: Go to Citizen ServicesDeathApply for New Registration.
  3. Register: Enter your mobile number and verify it using the OTP sent.
  4. Fill Form: Complete the application form with accurate details.
  5. Upload Documents: Attach necessary documents such as the deceased's ID proof, ration card, voter ID, and hospital discharge certificate (if applicable).
  6. Submit: Submit the application. An acknowledgment number will be provided.
  7. Follow-Up: Visit the respective hospital or municipal office with the acknowledgment number to collect the certificate.

For Certificate Verification

  • QR Code: Scan the QR code on the certificate using the portal's verification tool.
  • Certificate Number: Enter the certificate number along with the date of birth (for birth certificates) or date of death (for death certificates) to verify
